The Community Health Worker works with patients, care teams and community resources to coordinate and support chronic disease management and wellness initiatives through education, coaching, prevention initiatives and referrals.
Ideal candidate will demonstrate a command of customer care and service delivery, have an understanding of emotional intelligence while relating to patients, providers, and other team members.
The ideal candidate will demonstrate a professional demeanor and have excellent judgement, be organized, motivated and focused to work independently, keep accurate records as well as complete reports and meet deadlines in a timely efficient manner.
Heavy phone work and data keeping.

Qualifications:
MUST be fully vaccinated against COVID-19 High School diploma or GED required Ability to speak other languages preferred Experience working with underserved and diverse populations in community settings Experience in navigating health and social service systems Demonstrated ability to support, coach and motivate patients to reach their goals Additional certifications and education in community health is encouraged Experience in navigating electronic health record systems, data entry and phone work Job Type:
