, has a great opportunity for a Full Time Referral Coordinator.
The position will be based in our Temperance office.
$300 Sign-On Bonus Monday - Friday, no weekends, evenings, or holidays.
Description:
1) Maintains ongoing tracking and appropriate documentation of referrals to specialists and outside agencies 2) Perform patient needs assessments; assist patients in problem solving potential issues related to the health care system, financial or social barriers 3) Assess patient's social determinants of health (SDOH), acts as system navigator and patient advocate 4) Ensure patient's primary care chart is up to date with information on specialist consults, hospitalizations, and ER visits 5) Coach and motivate patients to engage in primary care to effectively manage their chronic disease or engage in preventive care as appropriate 6) Actively collaborate with other members of the primary care team including Care Managers, Support Staff and PCPs to address patient needs 7) Adhere to and support the policies and procedures of the Family Medical Center and recommend revision of policies and procedures as appropriate 8) Perform other duties as requested/assigned to include but not limited to insurance outreach and enrollment
Qualifications:
1) High School Diploma/GED 2) 2 years' experience in a clinical environment preferably as a MA, CNA, Medical Receptionist or similar 3) Must be well-organized, detail-oriented, and have the ability to multi-task in a demanding and constantly changing environment 4) Technologically literate; proficient in Microsoft Office software 5) Experience with eClinicalWorks electronic charting software preferred Family Medical Center of Michigan is an EEO/AA/ADA/D/V Employer Job Type:
